What are Connected Core's founding beliefs?
- Click the button above to schedule online or call 920-547-2717. Home-based initial evaluations last 90 minutes with follow-up sessions typically 60 minutes in duration. Clinic-based initial evaluations last 60 minutes with follow-up sessions typically 60 minutes in duration. Shorter/longer appointments are available upon request & can be discussed.
- It is encouraged to schedule 1x/week for 4 weeks to ensure availability of the time that works best for you as well as to help avoid gaps in care. It is very easy to cancel these appointments at the time of your evaluation should you not need them.
Do I need a referral to start care with Connected Core?
- Wisconsin is a direct access state; a referral form a doctor or other healthcare provider is NOT required to see a physical therapist. If you have recently given birth, had medical illness or hospitalization we may ask for proof of medical clearance or permission to communicate with your health care team for participation in physical therapy.
Do you accept insurance?
- Connected Core is a concierge private pay practice. Insurance is not accepted, nor do we submit Superbills to any third parties. A Superbill can be provided upon request for you to submit to your insurance if you would like to try to receive partial reimbursement for the services. Connected Core does not perform prior-authorizations or correspond with any third parties regarding reimbursement.
- Please note for Medicare beneficiaries, as a non-enrolled provider, you are not able to submit your receipt to Medicare as they will not reimburse services by a non-enrolled provider. Connected Core is not an enrolled provider with Medicare and provides wellness-based services only.
- We have elected to pursue this model of care for the following reasons: treat holistically, be proactive & preventative, find & address the root cause, have more one-on-one time together & provide the highest quality care you deserve while advocating for fair reimbursement that aligns with the highest quality certification/training & clinical expertise.
What forms of payment do you accept?
- Cash/Check/Credit cards/Flex spending cards/Health savings account (HSA) cards. You are able to submit paid receipts to Flex spending with full reimbursement. Superbills are available upon request for you to send to your (Non-Medicare) insurance company for partial reimbursement after your appointment.
What to expect from in-home sessions?
- Your Provider will bring the supplies needed for each session (treatment table, linens & clinic supplies). You will be emailed an intake form and details of your scheduled appointment. Please complete the paperwork prior to your scheduled appointment to allow your provider time to review. Please dress comfortably (T-shirt & shorts is great).
- Each session begins with thorough conversation and re-evaluation to best determine the priority of your needs. Your session will always be one on one with the primary focus on your healing. Treatments can consist of a variety of modalities including exercises personalized, breathing, hands on manual therapy, visceral mobilization, myofascial release, breath work, guided relaxation and internal pelvic floor work if this is appropriate for you. You will receive a personalized plan of care and guidance on exercises and/or lifestyle modifications as indicated.
What to anticipate at your first appointment?
- You should feel completely comfortable at every stage, understand what’s going on (and why)
- You should leave feeling much more knowledgeable, having a better understanding of what is going on and with a solid plan to improve. Connected Core wants you to feel much more hopeful when you leave and excited to work together.
- If you are seeking care for pelvic health conditions, during pregnancy or postpartum you can discuss the value of an internal pelvic floor evaluation. An internal exam is not required if you’re not comfortable with it, and typically is not done on the first day (unless requested). However, it is the gold standard of care and if you’re not getting an internal exam you’re missing a huge piece of your puzzle.
What do I wear?
- Many of you are wondering “what do I wear to a physical therapy visit?” In an ideal world you’d wear something comfortable, but it honestly doesn’t matter, especially for the initial evaluation. Jeans or rigid fabrics can create external restrictions in your mobility so softer, flexible fabrics can be more comfortable.
Do I take my clothes off?
- Wellness Clients-No. We occasionally will move your clothing to access skin for hands on manual therapies and dry needling, but this is done only after your consent and with your assistance.
- Pelvic Health-Yes...maybe...eventually, but not right away! Once you arrive you’ll first talk with your provider in a private treatment room. You’ll sit down - still completely clothed - and go through the intake paperwork and get a total picture of what brought you to PT. You should have enough time to completely explain what’s going on and not feel rushed at all - very different than a usual doctor’s appointment.
- Your pelvic PT will usually start with doing a thorough evaluation, seeing how you move, bend and stand. They’ll look at your hips, your back and posture, and how you’re breathing. This will be both on and off the treatment table. Often this is more than enough to assess during the first visit.
- Typically, at the 2nd or 3rd visit you will discuss the value of an internal pelvic evaluation. If agreed upon, your provider will then leave the room to allow you to undress from the waist down. You will then cover your body in a sheet and you’ll be covered as much as possible during the rest of your exam.
What's the internal exam like?
- This is not required for pelvic health care and can be further discussed with your provider.
- Your PT will explain exactly what’s going on, and then - as long as you’re completely comfortable - generally proceed to the internal portion of the exam. It will involve a single gloved finger evaluating the muscles of the pelvic floor. This should not be painful, and your physical therapist will make sure you’re informed and comfortable throughout the process. Afterwards your PT will leave the room, allow you to change and then return to discuss what was learned and your plan going forward.
Can I still attend pelvic physical therapy care if I have my period?
- Yes, having your period is not a problem for pelvic health care. Internal work can still be done based on your comfort and preference. If you prefer to not have any internal work done during your cycle, there are many other things to work on that will benefit your care.
Can I bring my children to my appointment?
- Yes, the current clinic space has room where children are welcome. Due to the clinic being in a quiet space with massage therapy nearby it is requested that children be occupied with quiet activities.
- Infants are welcome and many treatments allow for concurrent holding and breastfeeding.
- At this time there is not any administration staff available to assist with holding, watching or entertaining kiddos.
How many visits will I need?
- Each course of care is as unique as the individual attending. With that, we believe that even a single appointment can create empowering positive changes for you and tremendously improve your healing trajectory & well-being. We provide guidance on your individual findings and needs to work with you for your best possible outcomes.
- Typically, wellness, prevention or maintenance services are 1 to 2 visits per month.
- Care to address specific symptoms & functional impairments typically benefits from visits every 1-2 weeks for 8-12 visits total over a course of 3-6 months.
- Discounted packages are available for 6, 8 or 12 clinic visits.
- It is encouraged to schedule 3-4 weekly appointments at the time of scheduling your initial evaluation to prevent gaps in care. Your appointments can be cancelled if they are not needed, and your treatment plan will be discussed at your initial visit.
What if I can't find a time to schedule?
